Pediatric Pulmonary Case Manager

Traci Bell, RN, BSN, CPN is the CF Nurse Case Manager at Kaiser Permanente Pediatric CF Center in Roseville, Ca where for the last 7 years has worked with children with all pulmonary problems including, Cystic Fibrosis, Asthma, Bronchopulmonary Dysplasia, Neuromuscular disease, primary ciliary dyskinesia and who are ventilator/trach dependent. Traci began her career in 1992 as a Pediatric floor nurse at the UC Davis Medical Center in Sacramento which began my journey with Cystic fibrosis care in the hospital. During my 16 years taking care of patient's in the hospital setting, I could witness the strength and courage of my patients with Cystic Fibrosis. Traci joined Kaiser Permanente in 2008 as an Assistant Nurse Manager for the PED/PICU in the hospital. Traci returned to pediatric floor nursing in 2010 and in 2011 was hired by the Pulmonary department as the Pulmonary Case Manager in the outpatient setting. As the Pulmonary case manager, Traci coordinates the multidisciplinary CF visits, provides education, tracks necessary testing, lab work and medications for each patient. Traci is available to her patients and their families for advice calls, scheduling of appointments and ordering equipment and supplies. Since joining the pulmonary team, Traci has attended NACFC every year and has participated as a speaker, a session moderator and she has hosted a round table discussion both last year and this year. She is currently serving her last year as a nursing representative on the CFF Patient Education Committee. Traci volunteered to participate in a FUN LLC in 2016 where she worked with her CF center to improve OGTT testing compliance and motivating patients to obtain the annual testing. In the local area, she has served as a team leader for our CF Center's Great Strides team and has been a member of the Sacramento chapter board since 2012.
